| (a) Purpose of Collection |

The personal data and other related information provided by you by means of this form will be used by the Hong Kong Curators Association (Association) for member registration purpose. The provision of personal data and other related information by means of this form is voluntary. However, if you do not provide adequate data, the Association may not be able to process your application for membership.

| (b) Transfer of Data |

If the Association has accepted you as a member, your particulars, i.e. the personal data and other related information you provide by means of this form, will be compiled in a register and made available for inspection by the Registrar of Trade Unions. Where necessary, the particulars submitted by you may also be provided to other departments or organizations for the purposes of voter registration and election under relevant Ordinances/Legislation and all purposes related thereto.

You have a right to request access to, and to request the correction of, the personal data and other related information you supplied in accordance with the provisions of the Personal Data (Privacy) Ordinance. Such requests should be forwarded to:
